IMPORTANT INFORMATION

  • All Covid-19 Vaccine Appointments for AstraZeneca and Pfizer must be booked via our online booking system, please DO NOT contact reception regarding Covid-19 bookings or information.
  • Lilydale Medical Clinic is now administering the Pfizer Vaccine to all our current patients as of 20th September 2021 from 12 years to 59 years who have NOT already had their first dose of the AstraZeneca Vaccine. We are not administering Pfizer for over 60 years of age.
  • We have COVID Clinics for eligible patients to book online. If you are eligible, please book by health engine to avoid calling the clinic. If you see that there are no appointments available, this means we are fully booked. Please continue to check back as these appointments are regularly updated to maintain our stock availability and not overwhelm our hardworking team. We suggest checking availability at a Government Hub as you will likely get an appointment sooner than we can currently offer.
  • GP's will not be giving the vaccine during their regular appointment times. Pfizer Vaccination Clinics will be held strictly on Monday, Wednesday, Saturday and Sunday 10am-12pm and 1pm-3pm.
  • AstraZeneca vaccine is available for Persons over the age of 18 years who do not have a contraindication to the AstraZeneca Vaccine are eligible to receive the AstraZeneca Vaccine.
  • Astrazenca Vaccination clinic is on Tuesday 9am-12pm
  • Appointments for the COVID-19 vaccine can only be made online for this practice.
  • Once you have confirmed your eligibility for the vaccine, book online with health engine and click on COVID Vax Dose 1 or Dose 2 appointment. Do not proceed using any other appointment for the COVID-19 Vaccine
  • If you have any questions about the vaccine, please discuss this with your regular GP prior to vaccine clinic and do not book into a COVID clinic until you are willing to consent to the vaccine.
  • You must be eligible for Medicare or Department of Veteran Affairs (DVA) to be vaccinated here. If you do NOT have a Medicare or DVA card, please book your vaccination at one of the Government Vaccination Hubs instead.

    • Immunisation Statement downloaded for date of appointment. Able to show on phone
    • Booked appointment for 2nd dose of vaccine
    • Update email address and consent to receive emails
    • Email from practice with post-vaccine information (links) and self-reporting document

ELIGIBILITY DECLARATION

  • You will need to confirm your eligibility to receive the vaccine, please complete the document found here, additionally, you will need to bring one of the following to your appointment:

  • A current work ID card showing employment at a relevant occupation for Phase 1A or 1B
  • A letter from your employer confirming that you are currently employed in a priority occupation for Phase 1A or 1B
  • Documentation confirming you are a carer
  • For household contacts of a quarantine / border worker, proof of occupation of the quarantine / border worker and evidence to demonstrate you live at the same residential address. For instance, this could include: bills or rates notices with both of your names and address, or separate documentary evidence that shows each of your names while showing the same address
  • Details to enable the vaccine provider to access your MyHealth Record, if you have one

INSTALLING GOV APPS

  • Before receiving covid or flu vaccine this year, you will need to show a current Immunisation Statement.

    The easiest way to do this is from your phone. Before you can download the statement, you will need to install the app on your phone. For instructions for how to download MyGov and add Medicare to your services, download Express Plus Medicare app, and download Immunsation Statement, follow the steps from slide 6 to 9 from the Patient Information Pack (download here)


BOOK SECOND DOSE APPOINTMENT

  • An appointment for the second vaccine will be scheduled at the time of your first vaccine
  • Please note that you will need to provide an Immunisation Statement for your second dose. Refer to the instructions on slide 9 from the Patient Information Pack (download here)
  • Please ensure you have updated the practice with your email address and consent to receive emails

REPORTING ADVERSE EFFECTS

  • The government has mandated reporting of any adverse events. Further information and reporting can be found at www.safevac.org.au
